The DON confirmed the facility continued Seroquel for dementia even though dementia was not an appropriate indication for its use, and the medication was later discontinued because the resident was not having behaviors. For another resident with diagnoses including intracranial injury, vascular dementia, traumatic brain injury, major depression disorder, mood disorder, generalized anxiety disorder, and impulse disorders, the physician's orders included Ativan 0.5 mg twice daily for restlessness/anxiety and Ativan 0.5 mg every 12 hours as needed for restlessness. The record did not contain a stop date for the as-needed Ativan. The quarterly MDS showed severely impaired cognition and indicated the resident received an antipsychotic and an antianxiety medication. The DON confirmed there was no stop date for the as-needed Ativan. A third resident with diagnoses including subarachnoid hemorrhage, dementia, anxiety disorder, and depression had an order for Ativan 0.5 mg every four hours as needed for anxiety, but the record did not include a stop date for the medication. The psychiatric progress note stated the plan was to continue the current psychotropic medication and that Ativan could be given at bedtime and every four hours as needed for anxiety, with GDR clinically contraindicated unless otherwise noted. A fourth resident with diagnoses including COPD, chronic respiratory failure, anxiety disorder, and major depressive disorder had an order for Ativan 0.5 mg every eight hours as needed for dyspnea or anxiety, and the record did not show the required 14-day stop. The DON verified the antianxiety medication did not have the required stop date.

Psychotropic Medication Monitoring and Orthostatic BP Documentation Failures

R41 had severe cognitive impairment and diagnoses including non-traumatic brain dysfunction, hypertension, PVD, hyperlipidemia, arthritis, Alzheimer's disease, stroke, and non-Alzheimer's dementia. R41 received risperidone 0.25 mg three times daily for hallucinations, and the EHR did not show evidence of side effect monitoring. R91 had severe cognitive impairment and schizophrenia and received olanzapine and risperidone for paranoid schizophrenia, but the EHR did not show evidence of side effect monitoring. The record also showed failures to obtain and document orthostatic blood pressures as ordered for R10, R41, and R91. For R10, monthly orthostatic blood pressure documentation contained identical readings across lying, sitting, and standing positions on multiple dates. For R41, some monthly orthostatic blood pressure checks were documented as refused, with no evidence the monitoring was attempted again later, and other entries showed identical readings in all positions. For R91, one monthly entry lacked standing blood pressure and pulse documentation, and another showed identical readings in all positions. Staff interviews confirmed orthostatic blood pressures should be completed monthly in lying, sitting, and standing positions, that identical readings in all positions were not expected, and that side effect monitoring for psychotropic medications was not yet in place.
PRN Lorazepam Orders Lacked Required Stop Dates

Administrative Nurse D confirmed on 06/03/2026 that this PRN lorazepam order did not have the required stop date, consistent with the facility policy limiting PRN psychotropic medications to no more than 14 days unless extended with documented rationale and a specific duration.
Unnecessary Antipsychotic Use Without Documented Indication

The DON also stated the resident should have indications for antipsychotic use and that this did not meet expectation.
