Summary
The facility failed to document accurate information on the daily nurse staffing sheets by omitting the census numbers for all four resident neighborhoods across all shifts for four consecutive days. Record review showed that the census fields were left blank on the daily posted nurse staffing information sheets for the first, second, and third shifts on each of these days. During interviews, the temporary scheduler confirmed that she had not been including the facility census on the daily nurse staffing sheets and was unaware of the requirement.
