Summary
A deficiency occurred when a resident, who was moderately cognitively impaired and on a regular diet, was served a piece of toast that was approximately 75% blackened and burnt. The resident was observed with the burnt toast on her plate during breakfast, and although she considered eating it, her roommate intervened, suggesting she should not eat it.
