However, the forms were signed by the DON rather than the attending physician or Medical Director, and the MAR showed the resident continued to receive Ferrous Sulfate in tablet form via PEG tube with no documented change to liquid form. A third resident with Crohn's disease and anemia had a physician order for Retacrit, and the pharmacy recommended adding Ferrous Sulfate. The DON documented that the physician was consulted, but the review was not signed by the resident's primary care physician, and the resident was not receiving and had never received Ferrous Sulfate.
