Failure to Use Gloves During Blood Glucose Testing

Summary

The facility failed to follow standard precautions during point-of-care blood glucose testing for one resident with diabetes who had intact cognition and orders to have blood glucose monitored four times a day. During observation, the RN prepared to perform the resident’s morning blood glucose test in the unit dining room after performing hand hygiene and gathering the resident’s dedicated glucose monitor, test strip, lancet, and alcohol wipe. The RN verified the resident’s identity and obtained permission to test him at the table, then inserted the test strip and placed the lancet and monitor on the table next to the resident. Without donning gloves, the RN cleansed the resident’s finger with alcohol, used the lancet to obtain blood, and then picked up the glucose monitor to obtain the blood sample. The RN carried the monitor, used test strip, used lancet, and used alcohol wipe back to the treatment cart with bare hands. She then removed the used test strip using the alcohol prep wrapping and disposed of it and the used lancet in the sharps container before performing hand hygiene with soap and water. The RN confirmed she had not used gloves and stated she had received infection control training and competency testing. The DON stated the expectation was for staff to follow facility policy and standard precautions, including use of gloves related to the potential to transmit blood borne pathogens. The facility policy for blood glucose monitoring directed staff to perform hand hygiene, apply gloves, and then obtain the blood sample.
