Incomplete and Untimely Resident Medical Records

Summary

The facility failed to maintain complete, accurate, and readily accessible medical records for 2 of 3 residents reviewed, R1 and R4. R1’s face sheet identified diagnoses of atherosclerotic heart disease, morbid obesity, depression, and anxiety, and the MDS indicated R1 was independent with transfers and cognitively intact. During interview, the HIM stated R1 had routine nursing home visits on 8/1/25 and 10/7/25, but the physician visit dictated notes were not placed in the facility EHR because they were located in an outside medical system. The HIM stated she did not have access to that system and would need the DON to obtain the records. Review of R1’s EHR did not include those notes, although copies were later emailed to the DON and identified that R1 had been seen for routine nursing home rounds. R4’s face sheet identified diagnoses of unspecified dislocation of the right shoulder joint, obesity, and schizophrenia. The HIM stated R4 had a physician recertification visit and a routine nursing home visit on 9/26/25, but the note had not been removed from the outside medical system and placed into R4’s chart until 11/14/25. The HIM stated she was responsible for uploading faxed visit notes but needed access to the outside system to obtain other notes, and she was unaware of the facility’s process for ensuring timely placement of notes into the EHR. The VP-CS stated staff could enter the outside EHR, extract dictated notes, download them, and place them in the facility EHR under the miscellaneous tab. The DON stated she was unaware the notes had not been placed timely and stated all resident visit notes should be placed in the EHR as soon as available; she also stated that without the notes the resident’s medical record would not be complete.
