Summary

The facility failed to comprehensively assess and monitor a pressure wound for a resident with hemiplegia affecting the left side, diabetes mellitus, heart failure, bowel and bladder incontinence, impaired mobility, and a Braden score indicating risk for pressure ulcers. The resident’s care plan identified skin integrity risks and included barrier cream, heel elevation, keeping skin clean and dry, lotion, a pressure-reducing wheelchair cushion, and a pressure-reducing mattress. A weekly skin audit identified a scattered open area to the buttocks, but the record did not show a comprehensive wound assessment with details such as location, measurements, pain, wound periphery, drainage, or odor, and there was no indication the physician was notified at that time. The resident’s physician communication form later identified a 2 cm by 2.4 cm scabbed area on the left buttocks and requested an order for dressing as needed. The record did not identify the type or cause of the wound. A progress note documented an order for Mepilex to the scabbed area on the buttocks, to be changed every 72 hours and as needed, but the treatment administration record transcribed the order as PRN only rather than scheduled every 72 hours plus PRN. The resident’s MAR showed the ordered wound treatment was not applied from 10/21/25 through 11/18/25. During interview and observation, the resident stated the sore on the left side of her bottom had been there for a while and hurt at times. An NA observed an open area on the left buttocks at a bony prominence, and an LPN measured it and stated it appeared to be a pressure ulcer because it was at a bony prominence. The DON stated she was responsible for tracking and performing comprehensive assessments of pressure ulcers but had not kept up with it, and later assessed the wound as a stage 2 pressure ulcer. The facility’s protocol and skin care policy required full assessment, reporting, weekly skin assessment, and treatment per MD order, but those actions were not documented for this wound.
