Failure to Obtain Orders Before Applying Wound and Skin Treatments

Summary

The facility failed to ensure treatments were used per standards of practice and only applied with a corresponding physician order for 2 residents with non-pressure related skin concerns. One resident had diagnoses including ovarian and thyroid cancer, morbid obesity, and an abdominal hernia, and was dependent for bed mobility and transfers while receiving hospice services. Her care plan identified her as at risk for impaired skin integrity, and a progress note documented a fluid-filled blister on the right lower extremity that was covered with a transparent dressing. The electronic health record did not show that the physician had been notified of the blister or that an order had been obtained for the transparent dressing. During interview, the RN stated she observed the blister and discoloration on the back of the resident’s right leg and applied a clear Opsite dressing over the blister, but did not notify the hospice agency or physician and did not obtain an order before applying the dressing. She stated she did not need an order and used nursing judgment to decide what dressing to apply. The DON stated the dressing was not part of standing orders and that it would be standard practice to contact the physician to obtain an order before initiating treatment. The second resident had diagnoses including a right lower leg laceration, diabetes mellitus, and morbid obesity. Her hospital discharge instructions and physician orders directed that the wound be kept clean and dry and that the doctor be notified if signs of infection were noted. During observation, the wound above the right knee had an undated foam dressing, a scabbed/scarred area, a small open Y-shaped area, macerated tissue, and two purple sutures still present in the wound. The LPN stated the sutures had been missed, may be the reason the wound was not healing, and that she had been applying skin prep, bacitracin, and a foam dressing without an order for some time.
