Summary

The facility failed to assess and develop non-pharmacological interventions to promote comfort for 2 residents who were receiving PRN pain medication. For one resident, the admission MDS indicated intact cognition, frequent pain that occasionally affected sleep, rehabilitation therapy, and day-to-day activities, and a pain rating of 2 out of 10. The resident had diagnoses including lumbar vertebral fractures, low back pain, spinal stenosis of the cervical region, fibromyalgia, polyneuropathy, and disease of the spinal cord. The resident stated that ice packs helped with pain but were not offered, and the care plan did not show evidence of offering or identifying non-pharmacological interventions before PRN medication use. The resident’s MAR/TAR showed PRN oxycodone orders for pain, administered 43 times for the lower dose and 14 times for the higher dose during the month reviewed. Pain scores were documented and medication effectiveness was recorded, but the MAR/TAR lacked evidence that non-pharmacological interventions were offered before administration. Progress notes from the same period also lacked evidence of non-pharmacological interventions being offered for pain. Interviews with nursing staff and management confirmed that non-pharmacological interventions should be attempted and documented, and that this was not documented for the resident. For the second resident, the significant change MDS indicated severely impaired cognition, frequent pain that occasionally affected sleep and day-to-day activities, and a pain rating of 2 out of 10. Diagnoses included end stage renal disease, malnutrition, acute kidney failure, alcoholic cirrhosis with ascites, heart attack, and chronic liver failure. The resident stated that staff did not offer ice packs or heating pads and that no non-pharmacological interventions were offered for pain management. Although the care plan listed non-pharmacological interventions such as heat, cold, repositioning, rest, music, family/friend time, and relaxation, the MAR/TAR and progress notes lacked evidence that these interventions were offered before PRN hydromorphone was administered.
