Failure to Base Turning and Repositioning on Comprehensive Assessment

Summary

The facility failed to ensure individualized turning and repositioning programs were based on a completed comprehensive assessment for a resident with impaired cognition, Alzheimer’s disease, diabetes, and a cerebrovascular accident who was dependent on staff for all ADLs, including eating and mobility, and was incontinent of bowel and bladder. The resident’s annual MDS identified pressure ulcer risk, and a Braden assessment scored the resident at moderate risk for pressure ulcer development. Although a formal assessment was completed, no clinical assessment was completed, and the resident did not have a turning or repositioning program in place at that time. The resident’s CAA worksheet identified triggers including incontinence, pressure ulcer risk, and dependence on staff for turning and repositioning, but the analysis section was not completed for existing pressure ulcer/injury or extrinsic factors. The worksheet also lacked family or resident input and did not include objectives in the care plan considerations section. Progress notes documented impaired skin integrity on the buttocks/groin, then an open wound on the sacrum, and later an open area on the butt crack measuring 1.5 cm x 0.5 cm. A new turning and repositioning intervention of every 2-3 hours was added to the care plan, but the record did not show how that interval was determined or that a comprehensive assessment had been completed to evaluate tissue tolerance over bony prominences in relation to the resident’s clinical status and risk factors. The resident’s Braden scores remained at moderate risk, and the wound progressed from a stage 2 pressure injury to a stage 3 coccyx wound. The care plan was revised to include turning and repositioning every 2-3 hours and as needed, but the record did not show a comprehensive assessment addressing the turning and repositioning program after the ulcer deteriorated. Staff interviews showed they knew the resident was up for meals and otherwise in bed and turned every 2-3 hours, but they could not explain how that schedule was determined or whether it was effective. The DON, RN staff, regional nurse consultant, and wound NP all described the wound course and interventions, but the facility could not document a formalized process or tool used to determine the repositioning schedule, and the policy reviewed did not address turning and repositioning programs based on comprehensive assessments.
