Summary

The facility failed to have an active and ongoing plan for reducing the risk of Legionella and other opportunistic pathogens of premise plumbing (OPPP), and it also failed to ensure nursing staff used appropriate PPE for Enhanced Barrier Precautions (EBP). On 08/04/2025 at 10:00 AM, the Corporate Life Safety Director stated that the Building Maintenance Supervisor had resigned the prior week and that he was trying to locate documents related to the facility’s Water Management Program (WMP). He produced a binder containing a 2022 policy and blank monitoring forms. Review of the binder showed there was no diagram or text description of the building water system, no risk assessment, no identified areas where Legionella could grow and spread, and no listed control points, measures, or limits. There was no evidence of control point monitoring, no evidence that the water management team was meeting routinely, and the last water temperatures documented in the book were from 8/2/24. The facility’s WMP policy, dated 2022, stated that an infection control risk assessment should be conducted, a Water Management Team should meet quarterly, potentially hazardous areas or devices should be identified, control points and limits should be established, activities and corrective actions should be documented in meeting minutes, and the program should be reviewed annually. The Administrator stated that only Maintenance was on the team and that the team had not met since she arrived in March. The Infection Preventionist stated she had no involvement in the WMP other than assessing residents for signs and symptoms.
