Summary

The facility failed to keep a resident free from the use of a physical restraint when a gait belt was left around R200 and his wheelchair during breakfast. R200 was an [AGE]-year-old resident with Alzheimer's Disease and dementia, and his MDS showed a BIMS score of 3, indicating severe cognitive impairment. The incident was identified during review of a facility-reported incident and involved R200 being observed in the dining room with the gait belt around his waist and wheelchair. RN D stated that R200 had been wandering, agitated, combative, and had urinated on another resident's room floor that morning. She said she walked him to the dining room with the gait belt on and that staff asked to leave it on so they could assist him better and help prevent a fall. RN D later realized the gait belt was around both R200 and his wheelchair and directed staff to remove it immediately, stating it was being used as a restraint. CNA E reported she saw the gait belt around R200 and later around the wheelchair, and CNA I stated she had been told the gait belt around the resident and wheelchair was considered a restraint and abuse. R200's wife stated she was not notified about the incident.
