Summary

Failure to provide appropriate pressure ulcer care and prevent new ulcers from developing was identified for a resident admitted with constipation, heart failure, diabetes mellitus, and hypertension, who also required substantial maximal assistance with activities of daily living and had moderate cognitive impairment. The resident had a facility-acquired stage II pressure ulcer that was unhealed and was documented as being at risk for additional pressure ulcers. The resident was observed lying in bed on a regular pressure reduction mattress, and no air mattress overlay was present at the time of the first observation. The roommate stated staff had not turned and repositioned the resident during the prior night and stated the resident never had an air mattress on the bed. During a wound dressing change, the resident displayed severe pain, including hyperventilating, moaning, and repeatedly stating, "I just want it to be done" and "Ouch! Oh God!" The RN performing the dressing change stated the resident was in pain and identified one large unstageable pressure ulcer and three smaller stage II pressure ulcers that were facility acquired. The wounds measured 10 cm x 7 cm x 0.6 cm at the coccyx, 3 cm x 1.5 cm on the right buttock, 1.2 cm x 1.0 cm on the upper left buttock, and 3 cm x 3.5 cm on the lower left buttock. The resident was sent to the ED the same day for evaluation of the ulcerations. Hospital records showed the coccyx ulcer was infected and associated with sepsis, elevated lactic acid, hyponatremia, hyperglycemia, urinary retention, and diastolic heart failure. Consultation documented gram positive bacteremia, MSSA infection, leukocytosis, and a deep decubitus ulcer with exposed bone, with the wound not yet debrided at that point. Surgical findings later described black eschar, surrounding stage II decubitus tissue, tunneling under viable skin, and a cavity requiring debridement. Facility documentation showed the wounds had progressed over time from smaller open areas on the buttock to black, foul-smelling, unstageable wounds, while weekly wound measurements were not found in the electronic record and a quarterly Braden scale was missing for the next quarter month. The care plan included a low air loss pressure mattress, but staff interviews indicated the mattress was not in use until after the resident had already gone to the hospital.
