Summary

Provide and implement an infection prevention and control program was deficient because the facility did not have an active and ongoing plan for reducing the risk of Legionella and other opportunistic pathogens of premise plumbing (OPPP). During observation, multiple water fixtures and plumbing components were found in conditions suggesting they were not being routinely used or flushed, including three dead end water lines at the kitchen hand sink on the cook line, the three compartment sink, and the single compartment preparation sink. The hot water handle was not on the kitchen faucet, which did not allow for use or easy flushing of the water line. Additional observations found a spa tub in the central bath surrounded by resident equipment and used for storage, with the Maintenance Director unsure whether it was used or flushed regularly. In the skilled hall soiled utility room, the hopper had no running water from the spray or the hot side of the overhead faucet, and the Maintenance Director was unsure if it was flushed. In the [NAME] Hall shower room, two of three shower stalls had items stored in them and appeared not to be regularly used, and the Maintenance Director was unsure whether they were used or flushed. In the [NAME] Hall soiled utility room, the hot water on the overhead faucet was not turned on, indicating a stagnant line. In the Birch Hall weight room, a shower stall was present in the back left corner, and the Maintenance Director was unsure if the shower spray was flushed regularly. In the Birch Hall soiled utility room, brown and discolored water came from the hot water fixture of the hopper faucet, and the spray to the hopper was not turned on, indicating a stagnant line. Review of the facility's Water Management Plan and related meeting minutes found only the previous Maintenance staff present and no risk assessment with a written or visual display describing the building water systems.
