Dignified Dining Experience Not Maintained During Meal Service

Summary

The facility failed to provide a dignified dining environment for five residents on the Specialty Care Unit when dinner trays were not delivered to all residents seated at the same time. During observation, the meal cart on the SCU had no other meals on it and staff reported that more meals still needed to be brought by dietary staff. At that time, Residents #35, #39, #42, #57, and #7 either had not received meal trays or were waiting for staff assistance with their meals. A second cart was later brought to the unit, and the remaining residents received their meals after a delay. Resident #35, a female with dementia, aphasia, anxiety, and confusion, was observed looking around the dining room while her table mate was nearly finished eating and later eating dessert before she received her tray. Resident #39, a male with anemia, dementia, PTSD, vitamin D deficiency, and mixed irritable bowel syndrome, and Resident #42, a male with traumatic brain injury, dementia, mild intellectual disabilities, seizures, and signs and symptoms concerning food and fluid intake, were seated together with another resident who already had a tray, and they did not receive theirs until the second cart arrived. Resident #57, a male with diabetes, dementia, kidney disease, GERD, edema, stroke, and anxiety, asked for his dinner after seeing others receive theirs and later left the room upset after stating he did not like what was served. Resident #7, a male with dementia, legal blindness, and hearing loss, had his tray delivered to his room and placed on his nightstand while he remained lying with his head in the opposite direction; he was later assisted with eating after telling staff he was hungry. Staff and the DON stated that residents seated at the same table should receive their meals together because it was a dignity concern when one resident was eating while another had not yet received a meal.
