Summary

The facility failed to ensure the comprehensive care plan was revised by the interdisciplinary team for one resident, who was cognitively intact with a BIMS score of 15 out of 15 and had diagnoses including diabetes and hypertension. The resident’s care plan, last revised on 4/26/25, included interventions for a geri chair while up to assist with comfort and positioning and to limit time up in the geri chair to promote healthy skin integrity and wound healing. However, the resident’s stage four sacral pressure ulcer had been documented as healed on 2/5/25, and therapy documentation on 2/24/25 indicated the resident was to trial a wheelchair. Staff later stated the resident had been using a standard wheelchair since shortly after the wound healed, but the care plan was not updated to reflect that change. During observations in September 2025, the resident was seen in a standard wheelchair and reported that the wheelchair was uncomfortable and caused backside pain, stating that the standard wheelchair hurt so badly after sitting in it for a while. The resident said he or she had previously used a reclining, padded wheelchair and would like one again. Staff interviews showed the CNA was unaware of the discomfort, the UM and nurse were unaware the care plan still listed geri chair interventions and acknowledged the plan should have been updated at the last care plan meeting, and the DOR stated the resident had been advanced to a standard wheelchair after the pressure wound healed but nursing had not updated the care plan. The social services note from the care plan meeting indicated the care plans were reviewed and interventions remained appropriate.
