Summary

The facility failed to serve the pureed meal items listed on the posted menu for three observed meals. The facility policy on Nutritional Services - Menu Planning indicated that standard and therapeutic diets are to be professionally planned and approved by a qualified registered dietitian in accordance with physician orders and current dietary allowances. However, during lunch on 9/3/25, the Food Service Director (FSD) served pureed diets beef in tomato, mashed potatoes, and spinach instead of the menu items of chicken piccata or spinach stuffed sole, parmesan orzo, and spinach. The FSD stated that the facility practice was to serve pureed diets foods that were up to five days old and not to puree the posted menu items for those diets. During dinner on 9/3/25, the cook served pureed diets ground beef in tomato, spinach, and mashed potatoes instead of the posted cheeseburger with caramelized onions or egg salad, steak fries, and carrots. On 9/4/25, the surveyor again observed pureed diets being served ground beef in tomato, mixed vegetables, and mashed potatoes instead of the posted breaded pork chop and gravy or broccoli and cheddar quiche, au gratin potatoes, and summer blend vegetables. The Dietitian stated that residents requiring a pureed diet should receive the same entree as the regular textured diets and was not aware the FSD was not following the menus. The FSD later said he did not understand that he should be serving the pureed diets based on the menu breakdown, and the Executive Director stated that the cooks and FSD needed to follow the menu for pureed diets and that the puree diet should be receiving the same meal as posted.
