Summary

The facility failed to maintain a safe and comfortable environment for residents and staff, as evidenced by several observed deficiencies. On the Chesapeake unit, a locked closet with a damaged door revealed an exposed latch visible from the hallway, and a soiled utility room was found to have cracked floor tiles in front of a drain.
