Summary
The facility failed to ensure that care plans for residents were reviewed and revised at least quarterly and as necessary to address changes in residents' conditions. Record review showed that for two residents, the most recent care plans were significantly outdated, with one last updated in March and the other in April, despite the current review taking place in October. During an interview, the Director of Nursing confirmed that the quarterly care plans for both residents were overdue.
