Summary
Facility staff failed to conduct required quarterly care plan meetings for a resident, as evidenced by medical record review and staff interviews. The resident was admitted in November 2022, and while quarterly MDS assessments were completed in March and June 2025, there was no documentation or evidence of corresponding care plan meetings during those periods. The last recorded care plan meeting for the resident occurred in December 2024. Interviews with the Social Services Assistant and the Assistant Director of Nursing confirmed the absence of care plan meetings in March and June 2025.
