Summary
The facility failed to report an injury of unknown origin within the required 2-hour timeframe to the regulatory agency, the Office of Health Care Quality (OHCQ). A staff nurse became aware of a resident with severe cognitive impairment who was found with a swollen, bruised left eye. The resident was unable to communicate the cause of the injury.
