Summary

The facility failed to implement its Enhanced Barrier Precautions policy for a resident with a PEG tube and ordered EBP. Resident #6 had physician’s orders and a care plan indicating enhanced barrier precautions, and the door sign stated that staff must wear gloves and a gown for high-contact care activities, including dressing, bathing, transferring, changing linens, providing hygiene, changing briefs, assisting with toileting, and device care or use such as a feeding tube. During observation, two CNAs provided incontinence care to the resident without wearing gowns, and an LPN disconnected the enteral feeding tubing from the resident’s PEG tube without wearing a gown. Staff later confirmed the gown was required for those tasks, and the DON and Infection Preventionist confirmed the omissions. The facility also failed to maintain reusable medical equipment in a sanitary manner for two residents. In one resident’s bathroom, an unidentifiable and uncontained toilet seat riser was observed lying directly on the bathroom floor. In another resident’s bathroom, an unidentifiable and uncontained bed pan was observed lying directly on the bathroom floor on multiple occasions, including with a brown unknown substance along the inside surface and later along the surface of the bed pan. Facility staff confirmed both items had been used by residents and should not have been placed directly on the floor; they stated the items should have been labeled and placed inside a bag for storage. In addition, laundry handling did not follow the posted procedures. A laundry aide was observed placing dirty laundry into washers without sorting the items first, and the washers contained a mixture of sheets, towels, bed pads, and resident clothing. The laundry aide confirmed she did not sort the laundry before washing and did not use the correct wash cycle according to the wall chart. The posted laundry charts stated that soiled linen should be sorted before loading and that different items required specific wash cycles, and the laundry supervisor and administrator acknowledged the laundry aide did not follow those instructions.
