The LPN confirmed the resident was receiving 40 mL/hr continuously, acknowledged signing the feeding as administered at 50 mL/hr when it was only infusing at 40 mL/hr, and stated she had not checked the rate before documenting administration. The DON confirmed the nurse should have verified that the tube feeding was infusing at the physician-ordered rate before documenting it as administered.
