Summary

The facility failed to maintain a clean, comfortable, and homelike environment for one resident. The resident, who was cognitively intact according to a recent BIMS assessment, reported dissatisfaction with the cleanliness of his room, specifically noting the presence of dead roaches throughout the area since the last pest control treatment 2-3 weeks prior.
