Failure to Use PPE During Contact Isolation and EBP Care

Summary

Staff failed to use the appropriate PPE while handling a mattress in the room of a resident on contact isolation precautions. Resident #90 had osteomyelitis and an order dated 08/08/2025 for contact isolation precautions related to MRSA. On 08/12/2025 at 1:03 PM, the resident’s room door was open and displayed a contact isolation precaution sign, and S10 Maintenance moved the resident’s uncovered mattress without wearing gloves or a gown. The mattress touched the maintenance worker’s shirt and pants while it was being moved. The ADON, the maintenance worker, and the DON all confirmed that gloves and a gown should have been worn in the room. Staff also failed to wear the appropriate PPE while providing catheter care to a resident on Enhanced Barrier Precautions. Resident #4 had an EBP sign above the bed indicating that a gown must be worn. On 08/13/2025 at 9:30 AM, S16 CNA drained urine from the resident’s urine collection bag into an output container and then emptied the container into the toilet without wearing a gown. The CNA stated she did not think a gown was required because the EBP sign was not on the resident’s door. The IP had stated that residents on EBP are identified by a yellow sign above the bed requiring staff to apply PPE, and the DON later indicated PPE should be worn for catheter care.
