Infection Control Lapses With PPE and Hand Hygiene

Summary

The facility failed to maintain an infection prevention and control program for two residents reviewed for infection control. One resident was admitted with diagnoses including chronic kidney disease, pyuria, and klebsiella pneumoniae/ESBL in the urine. A urine culture confirmed klebsiella pneumoniae/ESBL, and the resident was placed on contact precautions by physician order. The resident’s care plan also identified infection control precautions related to ESBL in the urine and included the use of PPE as indicated. Observation of the resident’s room showed a contact precautions sign posted on the door stating that a gown and gloves were to be donned before entering the room, and PPE was available on a cart in the hallway. Despite this, the Admissions Director was observed inside the room without PPE while giving the resident a beverage and adjusting the resident’s pillow. During interview, the Admissions Director stated she had entered only to check on the resident and did not think PPE was necessary because she was not providing direct care, but later acknowledged she should have worn a gown and gloves before entering the room. A separate observation during medication pass showed a CNA leaving one resident’s room without performing hand hygiene, then preparing and administering medications to another resident without washing or sanitizing hands before or after resident contact. The CNA stated she did not perform hand hygiene before preparing or administering the medications and did not immediately wash her hands after leaving the room because it was easier to do so in the next room. She also stated she saw no problem with exiting one resident’s room and entering another without washing or sanitizing her hands.
