Summary

The facility failed to ensure an environment free from accident hazards and adequate supervision when Resident 1, who had diagnoses including Alzheimer's disease, dementia, adjustment disorder, and anxiety, was able to exit the building without staff knowledge or supervision. The resident's admission MDS documented severely impaired cognition with a BIMS score of 3, wandering behaviors, and rejection of care. Care area assessments noted frequent wandering into other residents' rooms, inability to stop wandering, and the need for frequent monitoring and redirection.
