Summary

The facility failed to protect Resident #54 from the use of a physical restraint that the resident could not remove on her own. Resident #54’s MDS documented diagnoses of Alzheimer’s Disease, heart failure, and anemia, and the BIMS was not completed because the resident was rarely or never understood. During observation on 8/25/2025, a positioning device was seen behind the resident’s left side of her back under the fitted sheet. During a later observation on 8/27/2025, two CNAs assisted the resident into bed, and after she was lying down, one CNA removed a positioning device from the closet and placed it under the fitted sheet and pressure relieving device behind the resident’s left side of her back before leaving the room. The facility’s restraint policy stated that if a resident cannot remove a device in the same manner it was applied and it restricts the resident’s typical ability to change position or place, the device is considered a restraint. The DON stated that the wedge should not have been under the sheet.
