The Administrator acknowledged the PBJ report reflected inaccurate data, and the facility had no policy for accurate submission of PBJ staffing data.

The CFO stated the facility did not learn the file was rejected until a letter was received in the mail from CMS on 04/22/2026, and that a later attempt to resubmit the data was too late for Quarter 4 reporting.
