Summary

Surveyors observed that multiple residents were positioned in bed with their pants or jeans pulled down to their ankles or knees, exposing their briefs. This was confirmed for five residents during random observations, with staff present at the time. One resident, diagnosed with Alzheimer's Disease, was found in bed with sweatpants around his ankles.
