Summary

The facility failed to maintain clinical records that were complete and accurately documented for a resident with non-pressure skin conditions. Resident 67, who had diagnoses including legal blindness and edema and required substantial assistance with ADLs and transfers, had a 7/30/25 physician order to cleanse both legs and apply Xeroform, ABDs, and Kerlix one time a day for blisters. During observation, the resident’s legs were wrapped in gauze, and the resident stated there had been blisters on her legs but was not sure whether they were still present. On a later treatment observation, a small dark circular discoloration was seen on the left shin, and the ADON stated it used to be a blister. The record lacked documentation of the resident having blisters on her legs, and the ADON stated she had been monitoring the blisters but had not been documenting it. The facility also inaccurately documented meal consumption for a hospice resident who was NPO. Resident 11, who had diagnoses including rectal and liver cancer, colostomy, and intestinal obstruction, had an NG tube to low intermittent suction and an order for nothing by mouth. The care plan stated the resident was NPO except ice chips and small sips of water. However, the task record documented meal intakes on multiple dates even though the DON stated the resident had not eaten since prior to admission and that meal consumptions should not have been documented because the resident did not eat.
