Summary

The facility failed to provide a restorative toileting program to maintain bladder functioning for one resident who was reviewed for bowel and bladder continence. The resident had diagnoses including major depressive disorder, bipolar disorder, asthma, and seizure, and was documented as frequently incontinent on the Minimum Data Set (MDS), though another MDS assessment indicated bladder continence.
