Summary

The facility failed to ensure the resident environment was free from accident hazards. Two residents were identified in the deficiency findings, including one resident who had a documented BIMS score of 15 and was cognitively intact, and another resident who also had a BIMS score of 15 and was cognitively intact. Both residents had standing orders allowing independent pass privileges, and the facility’s own policies stated that sharp or dangerous objects, including scissors, were not allowed in resident rooms and that resident belongings and rooms could be searched for contraband or unsafe items. One resident sustained a self-inflicted laceration to the right wrist after using scissors that were in her possession. Staff responded when the resident pulled the call light and found bleeding from the wrist. The resident stated she was depressed because her sister had died and said she used scissors to cut herself. The scissors were handed over to staff after the incident. The resident was transported to the hospital and returned the same day with 2 stitches to the wrist. The resident later stated she had brought the scissors from a previous facility and kept them in her bag during admissions, and that staff checked her belongings but did not see the scissors. The second resident was found to have scissors in her room during a room search after staff became aware she had been hiding medications. Staff also found a box containing numerous medications in the room. The resident stated the scissors had come from a previous nursing home. In addition, the resident had been given duloxetine, but staff documented that she hid the medication under her tongue and later produced it during physician rounds. The record states the resident had a care plan addressing hiding and hoarding medications and other non-allowed items. Staff interviews reflected that the expectation was to remove hazardous items and to ensure medications were swallowed in the nurse’s presence, but the report documents that the room search was not thorough and that the scissors and medications were found after the concern was identified.
