Summary

The facility failed to perform hand hygiene while providing eating and drinking assistance to 4 residents reviewed for dining. R52 was admitted with diagnoses including Alzheimer’s disease with early onset, major depressive disorder, and GERD without esophagitis; the MDS documented that the resident was dependent on staff for eating and the care plan stated extensive assistance with ADLs. R57 was admitted with diagnoses including GERD without esophagitis, unspecified hearing loss, and several intellectual disabilities; the MDS documented that the resident was dependent for eating and the care plan stated staff assistance was needed during mealtime. During observations on 8/18/25, 8/19/25, and 8/20/25, V4 (CNA) was observed providing feeding assistance to R52 and R57 without using hand hygiene in between assisting the residents. On 8/20/25, V4 stated she should have performed hand hygiene in between assisting residents with meals. R8 was admitted with diagnoses including sequelae of cerebral infarction, vascular dementia, cognitive communication deficit, muscle weakness, other lack of coordination, and dysphagia; the MDS documented that eating assistance was dependent and the care plan stated the resident was to receive assistance at mealtimes. R68 was admitted with diagnoses including multiple sclerosis, moderate protein-calorie malnutrition, cognitive communication deficit, and muscle weakness; the MDS documented a BIMS score of 8 and that the resident was dependent and required assistance with eating, and the care plan stated assistance with intake was needed. On 8/18/25, V2 (DON) was observed assisting R68 and then R8 without completing hand hygiene before assisting either resident or in between residents; V2 was also observed rubbing his right eye, then handling R68’s bread with bare hands and attempting to feed it to R68, and later grabbing R8’s cup and straw with his right hand to try to give R8 a drink. V1 (Administrator) stated that staff should wash their hands before assisting a resident with eating and should perform hand hygiene if touching the face or anything, and V2 stated he supposed hand hygiene should be done before and in between each resident.
