Summary

Food was not stored, labeled, or temperature-monitored in accordance with facility standards. Surveyors observed the freezer and cooler temperature logs missing required temperature checks, and staff stated temperatures were supposed to be measured and logged twice daily. The freezer had been broken for 2 days, and the log showed the freezer as out of service and then defrosting on consecutive days. Staff also verified that the freezer thermometer read 58 degrees Fahrenheit, while the walk-in cooler and reach-in refrigerator temperatures were above the facility’s stated acceptable range. During the observation, frozen foods were found thawed in the freezer, including ground beef, turkey, turkey sausages, breakfast sausage, and ice cream. Four bulk packages of ground beef were sitting on a cart outside the freezer and another bulk package was on a freezer shelf. In the refrigerator, a carton of pudding had an expiration date of 7/16/2025, and salad, pudding, and open sausage were observed in the cooler without preparation, open, or expiration dates. The dietary manager stated the freezer had been broken since the prior day and that staff were trying to salvage frozen foods by placing them in the cooler. Record review and interview also showed inaccurate or incomplete documentation on the temperature logs. A dietary aide and dietary manager verified that one temperature entry had been filled in later because it was forgotten at the time it was supposed to be recorded. The regional director of operations stated frozen meat could be thawed and used within 7 days once placed in the cooler and that food could be left out of refrigeration for 2 hours without danger. Later that day, staff stated all meat products were discarded, and the freezer remained inoperable while repair work was underway.
