Summary

The facility failed to ensure that a resident's nasal cannula was labeled with the date it was changed. During observation, the resident was seen using a nasal cannula that was not labeled. A registered nurse checked the cannula and confirmed it was not labeled, and stated that the resident's assigned RN was responsible for the resident. Another RN stated the nasal cannula should be changed every 72 hours and dated with the date it was changed for accuracy, and that labeling was needed to prevent mucus accumulation in the nasal tubing for infection control. The resident, who had diagnoses including primary hypertension, dependence on supplemental oxygen, and type 2 diabetes mellitus, had an active order for continuous oxygen at 3 liters via nasal cannula for COPD. The resident's MDS documented severely impaired mental status with a BIMS score of 06. The care plan addressed complications with gas exchange and directed that oxygen be administered as ordered. The facility policy stated that oxygen tubing, humidifier bottles, and filters were to be changed, cleaned, and maintained at least weekly and labeled with the date, time, and initials of the staff completing the service.
