For R7, the face sheet did not document schizoaffective disorder, but the physician admission/follow-up documentation listed schizoaffective disorder and schizophrenia diagnosis findings, and a physician progress note linked quetiapine to schizoaffective disorder. R7’s PASARR notice documented no Level II required. Interviews with the Business Office Manager, Administrator, and Social Service Director stated that a new psychiatric diagnosis should trigger a new PASARR Level II assessment and that mental illness or developmental disability should trigger Level II review. The report also noted R9 had psychiatric diagnoses on the face sheet, but the PASRR outcome explanation stated no mental health diagnosis was known or suspected.

However, the EMR contained no documentation of habilitation coordination or independent living skills training notes. Interviews with the Habilitation Coordinator, MDS Coordinator, DON, Director of IDD Service, and Administrator confirmed there was no documentation after monthly visits, that the facility was not aware of when the coordinator visited, and that there was no system in place to monitor oversight or ensure the required annual and quarterly meetings were held.

She stated she did not recall whether the resident had been admitted under emergency admission or exempted hospital discharge, acknowledged responsibility for ensuring the PASRR process was completed, and stated the omission was identified during a chart update when the Level I PASRR screening was redone. She stated the Level II PASRR evaluation was completed later.
