Summary

The facility failed to ensure that a resident with epilepsy was free from a significant medication error involving Keppra (levetiracetam). The resident’s neurologist issued a prescription for two 250 mg tablets by mouth twice daily, totaling 500 mg twice daily, and the facility’s physician order sheet also documented levetiracetam 250 mg by mouth twice a day. However, the resident’s MAR showed administration of Keppra 250 mg twice daily from 9/1/2025 through the morning dose on 11/25/2025, and the LPN stated she gave one 250 mg tablet that morning and verified the MAR as matching that dose. During observation, a bubble pack for Keppra 500 mg was found for the resident, with instructions that one 500 mg tablet was to be given by mouth twice daily. The bubble pack had handwritten notation of "1/2 tab," which the LPN said meant one-half tablet per dose, equal to 250 mg twice daily. The DON later confirmed that the most recent neurologist order was for Keppra 250 mg with instructions to administer two 250 mg tablets twice daily for a total of 500 mg twice daily, and stated the current dose should have been 500 mg twice daily.
