Summary

The facility failed to serve meals at required temperatures and with acceptable palatability, appearance, and consistency for residents receiving food prepared in the kitchen. On November 17, 2025, the dinner menu included hot dog with hot dog bun, and at 4:27 PM the hot dog items were observed on the steam table in the kitchen. V3 stated the meal was ready for service at 4:45 PM and had already taken temperatures, but when V30 measured the items, the mechanical soft hot dog was 110.4°F and the pureed hot dog was 129.7°F. The facility’s policy stated hot foods should begin at 135°F or higher and that foods below 125°F at bedside/table-side service must be reheated to 165°F before serving. During breakfast meal service on November 18, 2025, room trays for residents on the first floor were observed without hot plates or thermal pellets, with only an insulated dome cover over the plate, and the hot cereal had no lid. The trays were placed in a rolling cart covered with a plastic liner. On November 19, 2025, V37 stated dietary staff sometimes gave coffee that was clear and not even black and that plastic plates were only given in the evening, questioning how food could stay warm. At 9:04 AM, V11 measured a test tray for the 1 South unit and found the Western egg casserole and hot cereal at 120°F, stating that if temperatures were taken after room trays were passed out they would be below 110°F and that the site did not have enough hot plates. During lunch meal service on November 18, 2025, the beef stroganoff with egg noodles appeared mushy with overcooked noodles and shredded beef, and the broccoli appeared overcooked and dull green. The dietitian stated hot foods at the steam table should be 135-140°F and palatability should be at least 125°F.
