Summary

The facility failed to ensure the menu met a resident’s individual nutritional needs and preferences for 1 of 3 residents reviewed for dietary services. The resident was admitted with diagnoses including diffuse traumatic brain injury, traumatic hemorrhage of the cerebrum, open wound of head, serotonin syndrome, delirium, and depression. The resident’s MDS showed a BIMS score of 15, indicating cognitive intactness, and the care plan identified a nutritional problem related to TBI and depression with interventions to encourage oral intake of meals and snacks and to explain and reinforce the importance of maintaining the ordered diet. The physician ordered a no added salt regular texture diet. On the evening meal in question, the resident stated he did not receive a meal tray and was instead given three peanut butter and jelly sandwiches later that night, although he would have preferred the chicken and French fries served for dinner. Dietary staff stated the tray was made and sent out but did not reach the resident, and the cook stated two tickets may have been stuck together. Nursing staff were aware the tray had been served to the wrong person, but dietary staff were not told the resident needed another tray. The dietary manager stated the sandwiches were not an acceptable substitution for the planned meal and that the resident should have been offered a starch, vegetable, and dessert. The facility policy stated staff will monitor residents’ food and fluid intake and offer appropriate meal substitutions when needed.
