Summary

The facility failed to ensure sufficient staffing to meet residents’ needs timely for 4 of 4 residents reviewed for staffing concerns. The report states that the facility had 70 residents and that multiple residents and staff described delays in call light response, toileting assistance, incontinence care, and showers because there were not enough CNAs on duty. The facility’s own assessment noted that staffing needs were based on the resident population and care needs, and the administrator stated the facility tried to have 5-6 CNAs on day shift and 4-5 on night shift. R13, who was admitted with spastic quadriplegic cerebral palsy and epilepsy and required substantial to maximal assistance with toileting and bathing, was reported by his roommate and by R13 himself to have waited about 1.5 to 2 hours after an incontinent episode before being cleaned up. R13 and his roommate stated he was not offered a shower on his scheduled shower day, was later told staff did not have enough time or enough staff to assist him, and did not receive the shower until later. A CNA who worked that night stated R13 waited about 30 minutes for incontinence care because there were only two CNAs for about 40 residents, and another CNA stated 30 minutes was too long to wait. R4, who had a moderate cognitive deficit and required staff assistance for toilet hygiene and toilet transfer, stated call lights sometimes took up to 45 minutes to be answered and that he had been left soiled for up to three hours. R7, who was dependent for toilet hygiene and frequently incontinent of bowel, stated the longest his call light had taken to be answered was 45 minutes and that he had laid in feces for 45 minutes. R11, who required partial to moderate assistance with bathing, stated she did not receive her scheduled shower because staff said they did not have enough staff. Multiple CNAs and an RN stated staffing was not enough to meet residents’ needs timely, that call lights were not always answered timely, incontinence care could be delayed, and showers did not always get done.
