Delayed Incontinence Care and Call Light Response
Summary
The facility failed to ensure timely incontinence care and call light response for residents who were dependent on staff for toileting and hygiene. The report identified 3 of 3 residents reviewed for dignity concerns (R4, R7, and R13) who experienced delays in receiving assistance after incontinent episodes. Facility policies stated that incontinent residents would be checked approximately every two hours and provided perineal and genital care after each episode, and that resident call lights would be answered in a timely manner. R13 was admitted with spastic quadriplegic cerebral palsy and epilepsy, had a BIMS score of 15, and required substantial to maximal assistance with toileting. His care plan noted he refused to use a bedpan or toilet and that staff should respect his wishes and provide care as needed. R13 stated he sat in feces for almost 2 hours on one occasion. R14, who shared a room with R13 and also had a BIMS score of 15, stated it took approximately one and a half hours for CNAs to assist R13 after an incontinent episode because only two CNAs were working and they were busy with other residents. Two CNAs who worked that night stated R13 waited about 30 minutes for cleanup after a bowel movement, and both attributed the delay to limited staffing and competing resident needs. R4 had a BIMS score of 11, was dependent on staff for toilet hygiene, required substantial to maximal assistance for toilet transfer, and was occasionally incontinent of bowel and bladder. He stated call lights sometimes took up to 45 minutes to be answered and that he had been left soiled for up to three hours. R7 had a BIMS score of 15, was dependent on staff for toilet hygiene, and was frequently incontinent of bowel and occasionally incontinent of bladder. He stated call lights sometimes took 45 minutes to be answered and that he had laid in feces for 45 minutes. A CNA and an agency RN stated that with only two CNAs and multiple resident care demands, it could take up to 30 minutes to answer call lights and provide incontinence care, while the DON stated the facility had enough staff to meet resident needs timely and the Administrator stated call lights and incontinence care should be provided as quickly as possible.
