Summary

The facility failed to investigate an allegation of potential abuse involving one resident. According to the facility's own Abuse Prevention and Reporting policy, any incident or allegation involving abuse, neglect, exploitation, mistreatment, or misappropriation of resident property requires an investigation. On 6/15/2025, a resident's family member and Power of Attorney observed that the resident did not have his jacket and was cold, and were informed by a Certified Nursing Assistant that she had taken the jacket because the resident was exhibiting exit-seeking behaviors and intended to keep it from him. This incident was documented on a Concern/Compliment Form, but as of 9/17/2025, there was no documentation of any investigation into the allegation.
