Summary

The facility failed to ensure that an allegation of abuse involving a resident was immediately reported to the Administrator and the State Agency, as required by facility policy. According to documentation, a family member and a Power of Attorney visiting a resident noticed the resident was without his jacket and was cold. A Certified Nursing Assistant informed them that she had taken the resident's jacket and placed it in the shower room because the resident wanted to leave the facility, and she was not going to return it.
