Summary
A deficiency occurred when a resident, admitted with chronic respiratory failure, hypoxia, and tracheostomy status, was observed to have soiled linens that were not changed over multiple days. The resident, who is cognitively intact and dependent on staff for activities of daily living and mobility, was found lying in bed with a pillowcase that had a large brown stain on two consecutive days.
