Summary

The facility failed to provide transportation services for a resident who required an oncology consultation for an abdominal/uterine mass found on hospital CT scan in June 2025. The resident’s family member reported that the resident missed her original oncology appointment in July because the facility did not make proper arrangements, and he was told the missed appointment was due to insurance coverage. The resident was later seen by oncology on 8/28/2025 and was referred to a gynecologist-oncologist, with a biopsy now required to determine cancer treatment.
