Summary

The facility failed to maintain a clean and sanitary environment in resident rooms, specifically regarding the condition of couches. During observations, three residents' rooms were found to have couches with visible stains: one couch had multiple dark stains on the seat cushion, another had multiple white stains on the seat cushion, and a third had multiple dark stains on both the seat cushion and armrest.
