Insufficient Nursing Staffing Affected Showers, Restorative Care, Transfers, and 1:1 Supervision

Summary

The facility failed to provide sufficient nursing staff every day to meet residents’ needs and to have a licensed nurse in charge on each shift. Surveyors found that residents were not consistently receiving scheduled showers, restorative nursing services were not being provided as planned, a resident requiring mechanical-lift transfers was transferred by one CNA instead of two, and a resident ordered for around-the-clock 1:1 supervision did not consistently have a sitter present. These findings were documented through observation, interview, and record review across multiple floors. One cognitively intact resident with paraplegia, central cord syndrome, and restless leg syndrome reported that restorative exercise services had stopped after the CNA schedule changed and the RNAs were reassigned to CNA duties because of short staffing. The resident stated that he had not gotten out of bed and exercised since the beginning of the month and reported increased back, knee, and ankle pain, increased leg swelling, and feeling weaker overall. Another cognitively intact resident with quadriplegia and conversion disorder reported that restorative arm and leg exercises and assisted turning had also stopped after the schedule change, and that spasms in the back and legs had returned and the upper body felt tight. Review of the restorative logs showed that one resident’s last documented RNA service was on 11/02/25 and the other’s was on 11/05/25, while the monthly restorative review still indicated the programs should continue. Two residents reported missed showers because CNA staffing was insufficient after the schedule change. One resident stated she had received only one shower in two weeks, and the shower log showed she received 1 of 4 scheduled showers since the staffing change. Another resident stated he had received only two showers instead of the scheduled three per week, and the shower log showed he received 2 of 7 scheduled showers since the staffing change. Surveyors also observed a CNA transferring a resident independently with a mechanical lift even though the care plan required a 2-person transfer, and the CNA confirmed she was alone because staffing was short. In addition, a resident with an order for 1:1 supervision due to wandering and elopement risk was observed without a sitter when the previously assigned staff member had already left and the replacement had not yet arrived. Staffing observations and schedule reviews on multiple units showed periods with only 1 to 3 CNAs for resident censuses ranging from 18 to 49, and facility leadership confirmed that the goal was to have minimum CNA staffing levels higher than what was observed.
