Summary

The facility failed to ensure ready-to-eat food was not touched with bare hands during meal assistance for one resident who was reviewed for weight loss. During a lunch observation, a CMA assisted the resident with feeding by picking up a cookie with her bare right hand and placing it in the resident's mouth, then picking up the cookie again with her bare right hand and placing it in the resident's mouth again. She kept her fingers on the cookie and continued giving the resident bites, with her finger and thumb pressed into the soft cookie. The facility policy titled, Meal Service, stated associates should not need to wear gloves when distributing foods or assisting residents to dine unless touching ready-to-eat foods. During interview, the CMA stated she had been taught not to use gloves during meal assistance and did not know what else she could have used to feed the resident the cookie. The DON confirmed staff should have used a fork, spoon, or gloves for the bite-sized pieces of food.
