Failure to Maintain Sanitary Equipment and Disinfect Shared Devices

Fortuna Rehabilitation And Wellness Center, LpFortuna, California Survey Completed on 11-13-2025

Summary

The facility failed to maintain sanitary conditions for a resident’s bedside commode. Resident 1 was admitted in December 2023 with diagnoses including muscle weakness and unstable gait and mobility. During observations in the resident’s room, the bedside commode was seen positioned next to the bed with a brown splattered substance streaked on the top and backside of the seat and on the armrests. A CNA acknowledged the commode was soiled and needed to be cleaned, and stated it should have been cleaned and disinfected after each use. The resident also confirmed using the commode and expected it to be cleaned and ready for the next use. The facility also failed to store the metal ice scoop on the mobile hydration station in a sanitary manner. During observations in the hallway and dining room, the scoop was seen exposed and accessible to anyone. The Infection Preventionist confirmed the scoop should have been kept in a plastic bag to protect it and the ice from pathogens and contamination. The DON acknowledged the scoop should not be left out in the open and stated it should have been kept in a clean, closed container when not in use. Staff also failed to disinfect the vital sign machine and equipment between use for five residents. A CNA was observed obtaining vital signs for one resident and then using the same machine for other residents without disinfecting it between residents. The CNA acknowledged not disinfecting the equipment between residents and stated cleaning medical equipment between patient contact is the expectation. The Infection Prevention nurse and DON stated staff were expected to clean or disinfect equipment between residents to prevent transmission of infections through indirect contact.

Short Summary

Staff failed to consistently follow EBP and hand hygiene for two residents who required high-contact care. One resident with an indwelling catheter and cognitive impairment did not have EBP followed during transfers, clothing removal, and pericare, and another resident with a suprapubic catheter and multiple pressure ulcers had staff wear PPE incorrectly, remove PPE in the room without hand hygiene, and continue care after emptying the catheter bag without changing gloves or cleaning hands. Interviews confirmed staff were expected to use gowns, gloves, and hand hygiene for these tasks.
